Olen D. Yoder (1275 S 1100 W, Middlebury, Indiana 46540) is submitting a notice of intent to the Indiana Department of Environmental Management of our intent to comply with the requirements of 327 IAC 15-5 to discharge storm water from construction activities associated with creekside lawn furniture located at 1275 s 1100 w, middlebury, indiana 46540.  Run-off from the project will discharge to rowe eden ditch.
